Kelly Ingram-Mitchell holds a graduate degree in physical therapy and an undergraduate degree in exercise and sports medicine. Her experience exceeds 20 years in prevention services and injury management. Kelly was executive director of a national therapy provider network and director of operations for over 20 outpatient therapy centers.

Kelly spent many years developing and implementing client-specific prevention and rehab programs, including post-offer testing, physical demand analysis, ergonomics, early intervention and onsite therapy models. She developed a Functional Capacity Evaluation (FCE) Certification Program in 2000 and was a professional trainer for many years. In 2015, Kelly co-partnered with ISPA for a new, comprehensive protocol to include disability and compensation. Her focus is on research and education, allowing marketplace leverage to best impact client results. Kelly recognizes the need for heightened education that will provide our profession with the tools necessary to service both pre- and post-injury claims. Her goal is to operate an organized, top-tier group of experts and professional partners who will utilize the tools provided to best service UHS’ current and future clients. She is a member of JSEB, RIMS, Women in Small Business Organization, Women in Trucking, and American Trucking Association.

Jill Baxter brings over 25 years of expertise in the fields of workers’ compensation and disability. The past 15+ years have been focused on the development and growth of a national work injury management network; she was instrumental in bringing the network from a regional to a national presence.

From day one with UHS, Jill has assisted employers in producing measurable results as they address their return-to-work obstacles. These results are achieved by reducing work comp claims costs through employer prevention programs and implementation of true therapy partners. Her primary role within the organization is overseeing our top-tiered Strategic Solutions team, organizing all project development, coordinating national account management, and promoting clinical education. She provides additional support to provider relations and operations.

Cindy brings over 20 years of experience in the outpatient rehabilitation network arena.  Over the years she served as Director of Human Resources, Administrative Manager with a staff of 15+ employees in her current role as Director of Finance.  During that time, she attained commendable skills in medical billing, account management, reporting, and a strong customer service background. Her 30+ years of operational experience informs her sense of urgency and gives her the ability to always to always see the big picture.  

As Chief of Finance, Cindy oversees the billing procedures, accounts payable and receivable, and oversees management of the billing staff.  She also works with the care coordination, provider relations and business solutions team to promote network growth. Cindy is an integral team member, proving instrumental to the continued growth and development of Unify Health Services.

Keiosha Callahan is a certified and state licensed Athletic Trainer specializing in sports psychology, orthopedic injuries, and occupational health. In her current role as Director of Onsite Services for Unify Health Services and Program Manager for Harley-Davidson, she oversees preventative healthcare clinics, executes special projects, and presents at symposiums and conferences throughout the country. Her previous experiences include case management of industrial athletes, fitness program coordination, event planning, public speaking, research, and post-injury care. Keiosha has created customized rehabilitation and fitness programs for injured workers, provided on-site and remote care for new injuries, and designed various custom health & wellness programs. Before working in an industrial setting, Keiosha was the Associate Athletic Trainer for a Division I University and worked with numerous professional and semi-professional athletes.

Keiosha is a Certified Ergonomic Assessment Specialist, Certified Sport Emergency Medical Responder, published researcher, mentor, and member of the National Athletic Trainers’ Association. She has completed the OSHA 30, and various competent persons trainings enhancing her knowledge of workplace safety. A California native and former athlete, Keiosha has always had a passion for Sports Medicine. She holds a Bachelor’s degree in Athletic Training and a Master’s degree in Psychology with a certificate in Sports Psychology. Her educational endeavors are far from over, as she has a passion for learning and honing her craft.

Anne Felts has been working in the field of injury prevention and physical rehabilitation for over 25 years. As the Director of Clinical Services & Innovations, Anne oversees client onsite injury prevention, workforce solutions, innovations, ergonomics, safety, education, and team support. Additionally, her department is responsible for Quality Assurance processes, Physical Demand Analysis (PDAs), as well as Post-Offer Pre-Employment Testing / Physical Abilities Testing (POET / PAT) implementation and management. 

 Anne holds a Bachelor of Science degree in Athletic Training from Berry College and a Master of Science degree in Sports Medicine from Georgia State University.  She began her career as a traditional Athletic Trainer working in the sports arena as an Athletic Trainer at the 1996 Olympic & Paralympic Games, the Olympic Training Center, as well as World League Professional Football camp, NCAA Wrestling National Championships, and Collegiate and High School athletics. She began as the Director of a physician owned outpatient rehabilitation clinic, assisting physicians with procedures, performing rehabilitation, and managing the rehab staff. She parlayed that experience in spine rehabilitation into an outpatient clinical position with a focus on rehabilitation of the injured worker, work conditioning and Functional Capacity Evaluations (FCEs). Eventually she transitioned into the full-time industrial setting, with on-site work at a major automotive manufacturer. Since then, she has continued her professional growth and education, into injury prevention and management of the worker.  

Her post graduate training and education has focused on: Ergonomics, Industrial Rehabilitation, On-site Injury Prevention and Rehabilitation Services, Job Site Analysis, Safety / Accident Investigation, Ergonomics and Risk Management, Clinical Program Management, and Wellness Education.  

Certifications/Professional Organizations:  Anne is a Certified Athletic Trainer (National Athletic Trainers Association); State Certified Athletic Trainer in Georgia and Tennessee; Certified Ergonomics Assessment Specialist III; and Functional Capacity Evaluator (JTech Medical).

Leah Hinote serves as the Director of Care Coordination. She manages a team of experienced customer service professionals ensuring that each referred patient receives timely and excellent care. Leah has been with UHS since 2016 and has played an integral role in shaping our care coordination department. Leah ensures that her clients’ needs are met efficiently and with a sense of urgency. She works with a network of professionals to match referred employees with quality providers in good standing.

Leah understands each of her clients’ unique goals and needs, and she guides her team in proving care tailored to her clients’ preferences.  Leah holds a Bachelor’s Degree in English Literature from the University of North Florida. Prior to joining the Unify team in 2018, Leah worked in early childhood education teaching Kindergarten.

Gary offers insight and a wealth of industry knowledge, having over 30 years of experience in health care and workers’ compensation cost containment services. His broad background consists of roles in management, sales, operations, and risk management. Gary is respected as a reliable resource and has established countless long-term relationships throughout the United States.

Gary has been with Unify Health Services, LLC. for more than five years. He was a former co-owner and Vice President of Solin USA Inc., a national case management and disability company. In addition to his experience as Regional Manager for Concentra Managed Care (now Coventry/Enlyte), he also managed the National Telephonic Case Management Unit for Kemper Insurance and was awarded Salesperson of the Year when employed with MDM Diagnostic Management. He began his career as a vocational case manager for the University of Miami Pain Program and then as a field case manager for Travelers Insurance Company.

Gary holds both a Master’s of Science degree in Psychology from the University of Kentucky and an Educational Specialist degree in Counseling Psychology and Rehabilitation from the University of Missouri.

Born & raised in our headquarters of Jacksonville, FL, Allie joined UHS in 2015 and has served as our Ergonomics Program Manager, Advanced Services Manager, and Strategic Solutions Specialist before creating and leading our Customer Success department. Allie works closely with our clients and teams to identify best-fit solutions and overcome obstacles, oversees program rollouts, and shares opportunities for program growth. She is passionate about job safety and employee health. As a Certified Ergonomics Assessment Specialist, Allie understands the importance of considering each employer’s unique work requirements and worker population when building a successful program.

Prior to joining UHS in 2015, Allie obtained over 7 years of experience in customer service working for Limited Brands, where she achieved top results in customer satisfaction. Her experience as a leader, team builder, business and brand manager drives her relationships with UHS clients and providers.  Allie holds a Bachelor of Arts degree in Political Science from the University of Central Florida.  She achieved honors in the major for her undergraduate dissertation on water management of The Everglades.

Carrie Manges is from Columbia, South Carolina and serves UHS on both the Innovations/Quality Assurance team as well as the Strategic Solutions team.  Carrie has been a licensed clinician for more than 13 years, including 10 years in the Workers’ Compensation industry.  She holds certifications in Functional Capacity Evaluations and Impairment Ratings as well as Ergonomics. Carrie comes from an outpatient provider setting and was previously managing workers’ compensation programs in several states, performing CEUs, association lectures, and providing company-wide workers’ compensation education to minimize the gap in foundational knowledge of treating injured workers. 

With UHS, Carrie performs high level quality assurance for Functional Capacity Evaluation, Therapy cases, and Impairment Ratings.  She supports the Strategic Solutions team with clinical insight and client support. Carrie is passionate about broadening education and awareness surrounding therapy management of the injured worker to therapists and stakeholders alike, mentoring FCE evaluators, and providing valuable feedback to help in claims resolution.

Jessica has worked in the Physical Rehabilitation space for over 16 years, starting as a Physical Therapist Assistant and progressing to Director of Rehab. She has served UHS as a member of our Clinical & Recruiting Teams before transitioning to her current Human Resources leadership role.

Kelly joined UHS in early 2022 bringing 13 years of experience in the workers’ compensation industry working with employers, insurance carriers, captive groups, TPA’s and brokers. Her focus on helping clients reduce claims costs and injured employees recover and return-to-work makes Kelly an asset to our clients. She possesses strong client relationship skills with an emphasis on understanding client needs, creating strategies as a solution and maintaining a responsive and reliable approach.

 Kelly has worked with a diverse group of clients from coast to coast in the U.S. allowing her to grow a healthy knowledge base beneficial to her clients. She has presented at workers’ compensation symposiums and boards, developed program implementation strategies, and garnered extensive experience working with clients who have unionized employees. Her ability to provide recommendations and support to clients navigating internal hurdles regarding work comp and safety initiatives is a valuable benefit she brings to her relationships.

 Kelly’s wide range of experience includes marketing, sales, and sales management. She is an awarded salesperson with sales training experience and has been respected as a sales mentor and leader. She is a dedicated professional who is motivated in serving as a trusted advisor to her clients.
