Additionally, it provides a clear “Return to Work” plan for the patient.

 Services were designed with both client and claimant in mind. The clinic selected receives data that is useful in creating an appropriate return to work plan of care based on an intake discussion with an Adjuster/NCM. This may include FROI (First Report of Injury), JD (Job Description), PDA (Physical Demand Analysis) and clinical instruction from Unify team.

 by a PT or OT will ensure that the claimant is receiving medically necessary care, is progressing on an upward trajectory, is in compliance and remains motivated. If areas of concern, restrictions or delays are noted for consideration, then the clinical check point therapist will notify the referral source and work with members of the provider team to modify care.
