Our goal is to make the referral process as simple as possible whether you are a new customer or longtime partner. To place a referral, simply email, call or fax to us the claimant’s demographics, claim number, contact information, prescription, service type and any special instructions so that we may place the claimant in the fastest and most efficient way possible. Our care coordinators will quickly respond and take it from there!
The referral process will provide quick and easy access to our national network of expert providers. We can coordinate one person at one location or thousands across the country.
When you contact one of our experienced customer care representatives, you will receive prompt, courteous communications. Unify is focused on working collaboratively with customers, patients and providers to facilitate timely scheduling of services.
We will select the provider that best fits the needs of the claimant. We will locate a provider if the need is in an area that is remote or where we do not have coverage.
We will manage all aspects of treatment and communicate back all appointments, compliance updates and discharge plans. This saves you time and money!
